New Patient
Evaluations:
Please bring medical records with
you if available.
Please come to the office 30 minutes before your visit to complete our paperwork. Alternatively, you can download
all of our forms and either email, fax or bring them in with you to your visit.
Many times we are asked to see new patients that want
to "interview" the doctor or come in to "establish" but do not have any active problems. These visits
may or may not be covered by insurance. We will make every attempt to appropriately bill your insurance company or Medicare.
However, if the service is deemed non-covered by your insurance company or Medicare then you will be responsible for the charge.
Physical
Exams for new patients:
It is the policy of
our office to evaluate your needs in your initial visit and not perform full yearly physical examinations at that time.
Often we run into questions, additional tests that should be ordered, insurance issues or other problems. We will
be happy to evaluate your needs and then order the appropriate tests and schedule a comprehensive exam.
Follow up visits:
Follow up visits will be scheduled after your visit with our office. At times
we may call you to schedule after we obtain records or lab reports. Generally, we do not discuss abnormal tests on the
phone and request a visit to discuss the findings and treatment plan.
Sick visits:
If you are an established
patient we make every attempt to get you an appointment right away. Often we will see you the same or next day. If the
problem is an emergency then we ask that you call 911 and go to the nearest hospital. The physician in the emergency
room can contact our office if needed.
Medication
Refills:
If you need a medication refill please
allow 24-48 hours. Try not to wait until the last moment to have your medications renewed. We will not renew medications
during our off hours.
If you have not been in our office for 12 months
you will be required to make an office visit before a renewal is given.
If
you are calling about a medication that another physician has prescribed and want our office to refill it you will have to
make an appointment so that our office can review the medication for necessity and potential drug interactions.
Medications will not
be given for one year at a time. They will, however, be renewed for 6 months in most cases. This allows for appropriate
followup for the condition treated.
Late
for a visit?

and let us know how late you are going to be.
It may be necessary to
reschedule your appointment so that other patients can continue to be seen at their scheduled time.
Cancellation policy:
Please call our office 24 hours prior to your scheduled appointment. If within 24 hours we reserve
the right to charge you $50.00.
Missed appointments:
If
you miss your appointment we will call you to reschedule the first time. However, if there are additional missed appointments
there will be a charge of $50.00. We will keep a copy of a credit card on file for such occasions.
